The job

Lose fat, keep muscle on GLP-1

Losing fat while holding muscle on GLP-1 or tirzepatide is a two-part job: enough high-quality protein when appetite is gone, plus resistance training. Published incretin trials often report lean tissue as about 20–40% of weight lost — a literature range, not your forecast. The 2025 joint advisory from ACLM, ASN, the Obesity Medicine Association, and The Obesity Society puts 1.2–1.6 g/kg protein and structured strength work on the same list. Protein without training is not a lock-muscle protocol. PivotOrder prints this week’s working protein floor and three leucine hits so the plate still has a number when hunger is off. It does not measure muscle and does not guarantee preserved muscle.

What does lose-fat, keep-muscle actually mean on an incretin?

The job is fat down without spending lean tissue as the cheap part of the scale. On semaglutide or tirzepatide the drug can shut food noise off. Calories fall. Absolute protein often falls with them even if the protein percentage looks fine.

A bathroom scale cannot tell fat from muscle from water. DXA “lean mass” also includes organs, glycogen, and water. Skeletal muscle is the tissue you are trying not to donate. Treat any app number — including ours — as a floor for eating, not a scan.

What does the 2025 advisory actually say?

Four US societies published nutritional priorities to support GLP-1 therapy for obesity in 2025. Adequate protein and resistance training sit together. The protein band discussed for active weight reduction is 1.2–1.6 g/kg/day. Actual versus adjusted body weight is still debated; a clinician-set gram target wins when one exists.

The same papers are clear that raising protein alone is likely inadequate to support muscle without structured strength work. We operationalize the protein band. We do not replace the gym or the clinician.

How do I start this week if I am already on the shot?

Enter weight, drug, dose week, and what you can keep down. The card prints a working floor and three hits. If solids will not go down, dairy and eggs in a glass still count. Log what landed, not what you planned.

If you are pregnant, under 18, have kidney disease or a clinician protein cap, or have an eating-disorder history, stop and use the care-team number. Those cases are out of scope.

Questions people ask

Does hitting a protein floor lock muscle on Ozempic or Mounjaro?
No. A protein floor is an eating target. The 2025 joint advisory pairs protein with resistance training. PivotOrder does not measure muscle and does not guarantee preserved muscle.
Is lean mass the same as muscle?
No. DXA lean mass includes water, organs, glycogen, and other fat-free tissue. Skeletal muscle is contractile tissue. A scale cannot tell them apart.
What is the first thing to do this week?
Build this week’s plan from weight, drug, dose week, and appetite, then hit three leucine-checked plates. Your first week is free — the full daily loop, no card required. Then CA$15/month or CA$120/year.
