What to eat on GLP-1: a full day of small meals

On GLP-1 or tirzepatide, the eating question is not “what is healthy” — it is “what still fits.” Appetite and stomach volume both drop, so the day has to be built protein-first and volume-last: anchor each eating occasion on a dense protein that clears roughly 2.5–3 g leucine (the card’s working number is 2.7 g), add vegetables and fibre only after that anchor is in, and keep a cold sip option for the meal that fails. Three anchored occasions usually land an adult inside the 1.2–1.6 g/kg/day protein band the 2025 joint advisory from ACLM, ASN, the Obesity Medicine Association, and The Obesity Society discusses during weight reduction. Eating this way is not a muscle-preservation guarantee — protein is paired with resistance training in that advisory, and neither is measured by an app.

What does a day actually look like when appetite is gone?

Start with the anchor, not the meal. An anchor is the one food on the plate that carries the protein: eggs, Greek or strained yogurt, cottage cheese, chicken, fish, tofu, tempeh, or lean beef. Everything else on that plate is optional. If you eat the salad first and fill up, the anchor is what you lose — and the anchor was the job.

Portions shrink, occasions do not. Three smaller anchored occasions beat two normal meals you cannot finish. If breakfast has never worked for you, move the occasion rather than deleting it: a yogurt at 10am counts exactly the same as one at 7am.

Drink away from food. Fluid takes the volume a shrunken stomach needed for protein. Most people do better keeping most of their water in the gaps between eating occasions rather than with the plate.

Eat slowly and stop early. Early satiety on these drugs is abrupt rather than gradual, and pushing past it is how nausea starts. A plate you finish comfortably at 70% is a better plate than one that costs you the next occasion.

What should each occasion be anchored on?

Dense animal proteins and dairy clear a leucine hit at a much smaller volume than dilute plant plates do, which matters more than usual when total volume is the binding constraint. Plant-forward eating still works; it just needs concentrated sources — soy, seitan, lentils paired with grains — and often a slightly larger total.

The table below is direction, not prescription. Real gram counts depend on the exact product, and the card scores whatever you actually eat against this week’s threshold.

How much of this has to be protein?

The published band is 1.2–1.6 g/kg of body weight per day during active weight reduction. For an 80 kg adult that is roughly 96–128 g/day — which, spread across three occasions, is about 32–43 g each. That is a large number when a plate is small, and it is exactly why the anchor-first order matters.

A clinician-set gram target always wins over this band, especially in kidney disease, and routinely exceeding about 2 g/kg without medical advice is not the goal. More protein is not automatically more muscle.

Fibre and fluid still matter — constipation is one of the most common reasons people stall on these drugs — but they come after the anchor, not instead of it.

What about the days nothing works?

Some weeks the drug wins. On a sip-only or nausea day, one cold dairy-based hit is a good day, not a failed one. Log what landed rather than what you planned, and if sip-only becomes the normal week rather than the exception, recast the card so the target matches the volume you actually have.

If you cannot keep fluids down, that is a call to the prescribing clinician, not a protocol question for an app.

Questions people ask

What should I eat on Ozempic or Mounjaro when nothing sounds good?
Anchor each occasion on a dense protein — eggs, strained yogurt, cottage cheese, chicken, fish, or tofu — and add everything else afterwards. Cold and bland usually goes down easier than hot and aromatic.
How many meals a day should I eat on a GLP-1?
Three anchored eating occasions is the card's loop, because each one is a chance to clear a ~2.7 g leucine check. Timing is flexible; the number of protein anchors is the part that matters.
Do I need to count calories too?
Not for this job. Calories fall on their own when appetite is suppressed. Absolute protein grams are the number that quietly slips, which is what the floor is for.
Can I do this as a vegetarian or vegan?
Yes, with concentrated sources — soy, seitan, dairy or eggs if you eat them — and usually a slightly higher total, since plant proteins tend to carry less leucine per gram. The volume constraint just gets tighter.
