Ozempic, Wegovy, Mounjaro: why the scale can lie

People on Ozempic, Wegovy, Mounjaro, or Zepbound often watch the scale fall and still feel softer — arms, face, grip. Registration-trial DXA and later reviews show lean tissue can be a large share of incretin weight loss. Published figures often land around 20–40% of weight lost, with trial-to-trial spread (STEP 1 DXA analyses have been read near the high end; SURMOUNT-1 DXA nearer ~25%). That is a published range across studies — not your forecast. It is not your personal kilogram of muscle, not a photo diagnosis, and not a PivotOrder scan. Lean mass on DXA is not the same as skeletal muscle. The useful response is still the 2025 standard of care: adequate protein plus resistance training — executed on weeks when appetite is gone. We do not guarantee preserved muscle.

Why do people say they are losing muscle on Ozempic?

Appetite suppression cuts total food. Protein is bulky. Nausea makes solids fail. Rapid weight loss increases the share of lean tissue in what leaves. Feeling weaker, colder, or “empty” in the face is how that shows up before anyone books a DXA.

Those feelings are not a diagnosis. They are why the card leads with anxiety, then a number — not a calm calculator.

What did STEP 1 and SURMOUNT-1 actually measure?

Linge and colleagues (Circulation, 2024) reviewed muscle and GLP-1 receptor agonists in the context of adaptive versus maladaptive lean loss. Look and colleagues reported body-composition changes on tirzepatide in SURMOUNT-1 (Diabetes Obes Metab, 2025), with lean share near ~25% of lost weight in that DXA analysis.

We cite those papers. We did not run them. We do not translate a trial mean into your arms.

What should I do this week if I already feel soft?

Print a floor. Hit three plates or sips. Lift if your clinician says you can. Recast when dose-week, appetite, or weight move. Do not change the medication from an app.

Questions people ask

Does Ozempic or Mounjaro cause muscle loss?
Incretin weight loss can include a large lean-tissue share in published DXA work. That is not the same as a personal muscle diagnosis. Protein plus resistance training is the 2025 standard-of-care pairing.
Can a protein app stop muscle loss?
No app can promise that. A floor makes the eating job executable when hunger is off. Training and clinical context still sit outside the card.
Is tirzepatide different from semaglutide for lean mass?
Tirzepatide is a dual GIP/GLP-1 agonist. Semaglutide is a GLP-1 agonist. Both can quiet appetite. Neither licenses a muscle forecast from this product.
